The patient has opted out of the AHCIP. The patient has agreed to assume financial liability for all health services. Please contact your patient regarding payment for your services.
This patient is not eligible for coverage under the Alberta Health Care Insurance Plan, please contact the patient and inform them that there will be an invoice for services provided. Under the Canada Health Act all Canadian residents are eligible to receive physician services in Canada. The patient shall be charged the rates that are listed in the Schedule of Medical Benefits for services provided.
